New Client: You or your family member has just had an intake appointment with us.
Current Client: You have had 5 or more sessions with your individual therapist and/or 6 months of treatment.
Discharge/Transitioned Client: Your services have ended with GIFTS and/or you have transitioned to another community provider.
Post Discharge: Let us know how you’re doing more than 2 months after your services have ended.
